Dr. Gulseth is the Program Director for Anticoagulation Services at Sanford USD Medical Center in Sioux Falls, SD.  He is also an adjunct assistant professor at the South Dakota State University College of Pharmacy. 

Dr. Gulseth received his Doctor of Pharmacy degree in 1999 from North Dakota State University in Fargo, North Dakota. He completed residency training at United Hospital in St. Paul, Minnesota.

He is lead author and editor of Managing Anticoagulation Patients in the Hospital: the Inpatient Anticoagulation Service, published in 2007 by the American Society of Health-System Pharmacists (ASHP).  He is a co-editor and author of Anticoagulation Therapy: A Point of Care Guide, published in 2011 by the ASHP.

He is an active member of several professional pharmacy organizations, including the ASHP, the Minnesota Society of Health-System Pharmacists (MSHP), the South Dakota Society of Health-System Pharmacists, and the American College of Clinical Pharmacy.  He has served on the Editorial Advisory Board of the American Journal of Health-System Pharmacy and is a past president of MSHP.   He was elected a Fellow of the ASHP in 2011.   His research interests include evaluation of antithrombotic therapy safety and efficacy and evaluation of the benefits of pharmacist managed anticoagulation services.  He also is the President and owner of Michael P Gulseth Anticoagulation Consulting, Inc

Mr. Hoying graduated from pharmacy school at the University of Toledo. He then sought out advanced leadership training through a combined two-year residency in Pharmacy Administration and Master’s Degree in Hospital Pharmacy.  Upon graduation, Mike became Manager of Pharmacy services at Metro-Health Medical Center in Cleveland, OH.  Mike has been in his current role as Director of Pharmacy at Fairview and Lutheran Hospitals since 1994.

 

Mr. Hoying is recognized as an expert in regulatory standards.  Mr. Hoying has a strong interest in advancing patient care by leveraging the unique skills of the pharmacist.  Mike has presented at national meetings related to improving patient outcomes and patient satisfaction through advancing the pharmacy practice model. 

 

Mr. Hoying has a strong sense of professional responsibility and is committed to providing cost effective innovative care.  Mike is an active preceptor for pharmacy students and RPD for Fairview Hospital’s PGY-1 Residency.  Mike justified a PGY-2 residency in Psychiatry at Lutheran Hospital.

 

Mr. Hoying is a member of ASHP, OPA and OSHP and is a member of OSHP’s Professional Affairs and Economy committee.  Mr. Hoying serves as a PGY-1 surveyor for ASHP.

Dr. Riddle serves as Director of Clinical Development for Pharmacy OneSource/Wolters Kluwer Health; a company that provides cloud-based technology solutions and evidence-based content to support optimal quality in point-of-care delivery of health services. Steve is responsible for overall strategic direction around clinical content and determination of business opportunities for Pharmacy OneSource and Clinical Solutions. Steve also serves as the Director for the Fellowship Program and as liaison for the company to the healthcare profession.

He has 30 years’ experience in healthcare, including inpatient and ambulatory clinical services, pharmacy administration, quality improvement and informatics. Steve received his Bachelor of Pharmacy from Washington State University and his Doctorate of Pharmacy from the University of Washington. He is currently a Clinical Assistant Professor at the UW School of Pharmacy and serves on the Washington State University College of Pharmacy Dean’s Advisory Council. Steve’s specific professional interests center on quality improvement, health care system reform, population health management and wellness-centered healthcare.

He served ASHP as the Chair for the Section of Ambulatory Care Practitioners for the ASHP from 2011-2014. He current serves ASHP on the Center on Pharmacy Practice Advancement Advisory Committee, the PAC and Advocacy Advisory Committee and the Pharmacy Forecast Survey Panel. Steve currently serves on the Quality Metrics Expert Panel for the Pharmacy Quality Alliance (PQA) and as Co-Chair for the PQA Stakeholder Advisory Panel. Steve served as inaugural Chair for the Washington State Pharmacy Association’s Health Systems Academy. He is a member of ASHP, the American College of Clinical Pharmacists (ACCP) and the Washington State Pharmacy Association (WSPA) and supports these association’s goals of integrating and expanding pharmacy practice to improve patient care. Steve was named a Fellow of ASHP in 2009 and has received distinguished leadership awards from ASHP and WPSA.

Dr.  Stevenson is the president of hospitals and health systems services at Visante, Inc., in St. Paul, Minnesota. He serves as a Member of Clinical Advisory Board of Inform Med, Inc. Before joining Visante, he was chief pharmacy officer at the University of Michigan Health System and associate dean and department chair of clinical, social, and administrative sciences at the University Of Michigan College Of Pharmacy since 1999. He also served on the Board of Directors of the American Society of Health-System Pharmacists. From 1998 to 1991, Dr. Stevenson served as Director of Pharmacy Services at West Virginia University Hospitals and from 1991 to 1999 was the Director of Pharmacy Services for the 8-hospital Detroit Medical Center/Wayne State University. Dr. Stevenson served as a Director of CDex, Inc. from December 15, 2010 to September 8, 2015.

As a longtime advocate for healthcare quality improvement, Dr. Stevenson has published widely on topics of Health-system pharmacy administration, medication safety enhancements, pharmacoeconomics, and outcomes research. He serves as the preceptor for the University of Michigan College of Pharmacy Pharmacoeconomics and Outcomes Fellowship. He has taken lead roles in evaluating the efficacy of numerous medication safety technologies including the use of spectroscopy for compounding validation and smart infusion devices. Dr. Stevenson serves as the medication safety section editor for the Joint Commission Journal on Quality and Patient Safety. His research interests are in pharmacy practice management, Pharmacoeconomics, and medication safety. He received Pharmacist of the Year awards from both the Michigan Society of Health-system Pharmacists and the Michigan Pharmacists Association. He received the Distinguished Alumnus Award from the Wayne State University Pharmacy Alumni Association and the Joseph Oddis Leadership Award by MSHP. He recently completed a term of service on the Board of Directors of the American Society of Health-system Pharmacists and is the recipient of the 2010 John W. Webb Lecture Award.

Dr. Stevenson received his B.S. in Pharmacy and Pharm. D. degrees from Wayne State University, College of Pharmacy and Allied Health Professions, Detroit, Michigan.

Professor Paul J. Declerck, obtained his Ph.D. in Pharmaceutical Sciences from the KU Leuven (Belgium) in 1984. After a post-doctoral training in the Laboratory of Biochemical Cytology (Prof. Dr. C. de Duve) at the Rockefeller University in New York he joined in 1986 the Center for Molecular and Vascular Biology (Prof. D. Collen) at the KU Leuven. In 1991 he was appointed professor of Pharmaceutical Biotechnology at the Faculty of Pharmaceutical Sciences. He became full professor in 1997. He is Research Director of the Laboratory for Therapeutic and Diagnostic Antibodies at the Department of Pharmaceutical and Pharmacological Sciences (KU Leuven). His research is focused on structure-function relationships of (recombinant) proteins and on the development of monoclonal antibodies for research, diagnostic and therapeutic purposes. He has expertise in the area of recombinant proteins, monoclonal antibody technology, biotechnology, drug    development, structure-function relationship in proteins, biosimilars.

Prof. Declerck has given numerous invited lectures at international meetings and has authored more than 230 scientific papers in peer-reviewed journals.

He is Dean of the faculty of Pharmaceutical Sciences of the KU Leuven, president of the International Society for Fibrinolysis and Proteolysis, member of the Commission of Medicines for human use of the Belgian Federal Agency for Medicines and Health Products, member of the Working Party on Gene Therapy Products (European Pharmacopoeia Commission, European Directorate for the Quality of Medicines) and member of various international scientific advisory boards.

Dr. Imad Treish is the Chief Operating Officer at the King Hussein Cancer Center  in Amman, Jordan. He has been overseeing operations in this 174 beds comprehensive cancer center since 2009. In his current role Dr. Treish oversees a variety functions including direct oversight of administrative departments as well as operational oversight of medical and nursing departments. Dr. Treish chairs the center’s Environment of Care committee which aims to achieve a secure, safe, and pleasant environment for patients, visitors, and employees. Furthermore, Dr. Treish is overseeing the transformation of KHCC to electronic medical records (Vista). Together with the CEO, Dr. Treish is directing the design and construction of the new towers for the cancer center. He is a member of the Executive Council, Strategic Planning Committee, Budget Committee, Medical Board, and Masterplanning Committee, and Construction Committee Prior to this position, Dr. Treish served as the Chairman of the Department of Pharmacy between 2002 and 2009. Dr. Treish has extensive experience in oncology pharmacy practice particularly in the fields of cytotoxic drug safety and environmental controls to reduce cross contamination of cytotoxic drugs. In addition, Dr. Treish has special interest in biomarkers and more specifically genomic markers of drug efficacy, toxicity, and economics.  Dr. Treish is a graduate from the University of North Carolina at Chapel Hill School of Pharmacy in 1999. He completed training in Hematology / Oncology at the UNC Healthcare in North Carolina in 2000.

Dr. Osama is currently the Senior Director of Department of Pharmacy Services at Cleveland Clinic Abu Dhabi since November 2010. Before joining Abu Dhabi, Osama was the Director of Pharmacy Services Administration at King Fahad Medical City (KFMC), a 1000-bed hospital, for 4 years. Osama also served as the Director of Pharmacies of Al-Rahba and Mafraq hospitals, Abu Dhabi, UAE for 3 years.

He also served at King Faisal Specialist Hospital (KFSHRC), Riyadh as a Clinical Pharmacist and Supervisor of the TPN Team and IV Room for 11 years. Before joining KFSHRC, Osama worked for 9 years at Al-Hada Military Hospital as Outpatient Pharmacy Supervisor and Drug Information Pharmacist.

He graduated as Pharmacist in 1984; he has an executive MBA and is Board Certified in Nutrition Support. He was a guest speaker in more than 100 symposia & seminars in topics related to Nutrition, IV Fluid Management, Pharmacy Technologies, and Quality Management. He had given more than 1000 presentations and has many publications and Performance Improvement Projects in the field of TPN, Fluid & Electrolytes Therapy, and Quality Management.
